William Penn has a new HIPAA form. The final date we will be accepting any previous version is November 6, 2024.
We recently updated our HIPAA form for the state of New York to LU1250NY (7-24). This new form is available on Partner Dashboard. We will not be accepting any earlier versions of this form after November 6, 2024.
What you need to know:
If you submit form LU1250WP (5-20) or any earlier version after November 6, 2024, it will be rejected and cause a delay in processing.
